 note = {肺高血圧症における右室駆出率(RVEF)は治療評価や予後予測に有用とされるが,心エコー図による評価は難しい.RVEF は心プールシンチグラフィやMRI により測定が可能であり信頼性も高い.本研究では心プールシンチグラフィによるRVEF と米国心臓超音波学会(The American Society of Echocardiography:ASE)より推奨されている心エコー図での右室収縮機能諸指標との関係について検討した.22 例の肺高血圧症患者において計29 回の心プールシンチグラフィと断層(2D)心エコー図を行った.RVEF は右室(RV) Teiindex と強い相関がみられた(r=-0.584, p<0.001).ROC 解析ではRVEF 35%を予測するRV Tei index のcut off 値は0.371 であった(AUC=0.768,感度=0.857,特異度=0.667).2D 心エコー図によるRV Tei index は肺高血圧症患者におけるRVEF を予測可能と思われた., Right ventricular ejection fraction (RVEF) is a useful marker to evaluate the effect of treatment and/or to predict prognosis in patients with pulmonary hypertension. However, it is difficult to assess the RVEF by echocardiography. Although measurements of RVEF is capable by specific modalities such as radionuclide cardio-angiography and/or cardiac magnetic resonance imaging (MRI), these modalities were not available in every medical centers. This study aimed to assess the echocardiographic surrogate markers to reflect the RVEF evaluated by radionuclide cardio-angiography. We compared the RVEF by radionuclide cardio-angiography with the echocardiograpic parameters that is recommended by the American Society of Echocardiography (ASE) in 29 measurements from 22 patients with pulmonary hypertension. As a result, radionuclide RVEF was correlated with echocardiographic RV Tei index (r=-0.584, p<0.001). I addition, receiver operating curve( ROC) analysis showed that 0.371 as the cut off value of RV Tei index could predict 35 % as that of RVEF (AUC=0.768, sensitivity=0.857, specificity=0.667). These results suggest that echo cardiographic RV Tei index is an useful marker to predict RVEF in patients with pulmonary hypertension., 原著},
