We present a case of impending rupture of an infected aortic aneurysm due to invasive pulmonary aspergillosis affecting an intralobar pulmonary sequestration. A 67-year-old man had acute lymphocytic leukemia and underwent chemotherapy, with complete remission. Previously, he had been diagnosed with asymptomatic pulmonary intralobar sequestration in the left lower lung. One month after chemotherapy, he complained chest and back pain, and was treated with antibiotics for one month. However, the pain continued and the patient was referred to our hospital. CT scan findings revealed an aortic aneurysm, which was diagnosed as an impending rupture of an infected aortic aneurysm with a maximum diameter of 9 cm, thus an emergency operation was performed. Aortic aneurysm was resected with both of a left lower lobe including the pulmonary sequestration and an aberrant feeding artery. Pathological study revealed invasive pulmonary aspergillosis originated from the pulmonary sequestration in left lower lobe. |
