"造血器腫瘍,再生不良性貧血などの血液疾患に合併する感染症は,原疾患による正常血球の産生障害と化学療法等による治療の結果生じた好中球減少時に惹起される日和見感染が主である.敗血症が疑われても,血液培養陰性のことが多く,起因菌不明のまま,抗菌剤治療を開始せざるを得ない.好中球1000/μl未満,腋下体温37.5℃以上の場合,febrile neutropeniaと呼んで一括して感染症として扱い,ガイドラインに沿ったemoiric therapyの施行が推奨されている.菌血症の起因菌は,グラム陰性桿菌からグラム陽性球菌主体に推移して来た.内臓真菌症の合併も多いが,やはり培養陽性となり難く,日本では血清学的診断法が普及している.ウイルス感染症は,造血幹細胞移植後30日以降など細胞性免疫能の低下時に,ヘルペスウイルス科など内因性ウイルスが再活性化され起こる回帰感染が多い.移植後では発症時期が予測できるため,抗ウイルス薬の予防投与や早期投与により,かなり対処できるようになった.また,血液培養陰性菌血症に対して,好中球中細菌核酸同定検査(ISH法)が開発され,その有用性が期待されている.",
