| 内容記述 | <目的>本研究では，リハビリテーション看護に関わる看護師のキャリア発達の支援を検討する基礎資料を得ることを目的として，看護師のやりがいについて明らかにする． < 研究方法> 西日本の回復期リハビリテーション施設9病院に1年以上従事する看護師を対象として自記式質問紙法のアンケートを配布した． < 結果および考察> 西日本の回復期リハビリテーション病棟従事1年以上の看護師146名中111名から回収を得た．その中から，やりがいに関する調査項目の記述があった59名を分析対象とした（有効回答率53.1％）．分析は，記述内容をデータ化し，類似性に従い分類し，カテゴリが形成されなくなった段階で分析を終了し，質的帰納的分析を行った．その結果，78のデータから13のサブカテゴリと5つのコアカテゴリが抽出された．コアカテゴリは，【個別性に応じた看護の実感】【患者の回復の実感】【家族を含めたスムーズなチーム医療の実感】【家族の患者理解と受入れによる喜びの共有の実感】【患者家族から向けられた信頼の実感】であった．【個別性に応じた看護の実感】【患者の回復の実感】【家族を含めたスムーズなチーム医療の実感】は，チームの連携による患者の回復を見ることで，看護実践の成果を確信でき，やりがいにつながるものと考えられる．この中で【家族を含めたスムーズなチーム医療の実感】は，リハビリテーションに関わる看護師に特徴的なやりがいと考えられる．さらに，【家族の患者理解と受入れによる喜びの共有の実感】【患者家族から向けられた信頼の実感】は，患者・家族と看護介入を共有したことや看護介入の成果の喜びを分かつ経験をして信頼関係が構築され，やりがいとして抽出されたと考えられる．患者・家族－看護師関係がやりがいに影響を与えることから，人間関係構築への支援がキャリア発達には重要であることが示唆された． |

| 内容記述 | Purpose: This study sought to clarify what aspects of rehabilitation nursing are important to nurse satisfaction in order to determine how best to support rehabilitation nurses’ career development. Methods: Subjects were 146 nurses working for more than one year in nine sub-acute rehabilitation hospitals in western Japan. Data were collected via a self-report questionnaire from May‒July 2008. Results and Discussion: Of 111 of the targeted 146 nurses who responded to the questionnaire, 59 completed all relevant questionnaire items（response rate 53.1%）and became the target for analysis. Qualitative and inductive analysis of the data was conducted. Following classification of similar descriptive contents into categories, 13 sub-categories and 5 core categories were extracted from 78 data items. Core categories were “providing nursing care tailored to individuals”, “enabling patient recovery”, “providing team medical care smoothly that includes the patient’s family”, “sharing joy with the patient’s family at their acceptance and understanding of the patient”, and “being trusted by the patient’s family”. The first three core categories are considered to lead to nurse satisfaction, where observing the patient’s recovery owing to team cooperation is seen as worthwhile by nurses, convincing them of the outcomes of their nursing practice. Particularly, “providing team medical care smoothly that includes the patient’s family” characterizes the satisfaction of nurses involved in rehabilitation. “Sharing joy with the patient’s family at their acceptance and understanding of the patient” and “being trusted by the patient’s family” lead to satisfaction, by forging a trusting relationship with both the family through sharing the experience of nursing intervention and the joy of its outcome with both patients and families. Providing support for nurses to build personal relationships with patients and their families is important for their career development since these relationships impact on what nurses feel to be worthwhile in their job. |
